Enrollment Forms

All of the forms that are required by the State of California as well as Little Saplings are made available to you here on this page in PDF form. These forms are required to be on site in your child’s file in order for your child to attend Little Saplings Preschool. To save time you may print and fill out the forms, with the exception of the Enrollment Agreement, and bring them in at the time of enrollment. 

The Enrollment Agreement is available for you to read on this page as well, however this will be covered and filled out with the parent(s) and the director.

All children must have immunizations. Immunization records required before the first day of attendance.
