Enrollment Forms

There are forms that are required by the State of California as well as Little Saplings that will need to be filled out and are required to be on site in your child’s file in order for your child to attend Little Saplings Preschool. After your first visit we will set up an enrollment appointment to fill out all forms that are required. 

The Enrollment Agreement is available for you to read on this page as well, however this will be covered and filled out with the parent(s) and the director.

All children must have immunizations. Immunization records required before the first day of attendance.
